Approval Process for Developing Existing Classroom Courses for the Online Delivery System - This handout is a step-by-step explanation of the procedure for developing an existing class for online use. Please download and read carefully.

Form A: Authorization to Develop an Online Course - This form is used to request permission to develop an online course for the first time. The course must already be approved for classroom use by the Dean's Council and Faculty. This form secures permission to convert the classroom course into the online delivery system. *Please fill out every blank of this form including your signature. The form may be returned if it is incomplete.

Form E: Online Course Revision Approval Form - This form is used to request permission to revise an existing online course. When substantial change needs to be made to a course (e.g., textbook changes, content revisions, course combinations, etc.), use this form for permission. *Please fill out every blank of this form including your signature. The form may be returned if it is incomplete.

Form D: Blackboard Guidelines for Internet Course Design - This is the scoring sheet that is used by the Review Committee. These are the items the Committee will look for in your course design. We recommend you download a copy of these to follow as you develop your course.

Form T: Authorization to Translate an Online Course - This form is used to request permission to translate an online course into Korean or Spanish for the first time. The course must already be approved for classroom use by the Dean's Council and Faculty. This form secures permission to translate the classroom course for the online delivery system. *Please fill out every blank of this form including your signature. The form may be returned if it is incomplete.

Generic Online Syllabus Template - This template is used to post your syllabus for the students to browse. It is housed in the Online Learning Center website. It contains no dates, no course schedule, nor instructor information. Students will use this to purchase textbooks and get a general feel for the contents of the course.
